Your first visit to Bloom & Bloom, DMDs involves a few special steps
so that we can get to know you. You can save time at your first visit by completing any necessary form prior to your
appointment. If you take medications or any over the counter supplements, please make sure to include them on your
medical history form. Providing us with this needed information will ensure that the doctor and staff are able to
provide you with the best possible care. .

We feel it is important for you to know that all appointments you make with Bloom and Bloom DMDs are reserved exclusively for you - we do not double book. Your first visit will likely be longer than your following visits and will consist of many information gathering procedures. This is to get to know you as a person and a patient, and to ensure we can appropriately assess your dental needs. To make this process efficient, please have your most recent full set of dental x-rays, recent bitewing x-rays, and/or recent panoramic x-rays forwarded to our office prior to your appointment. If you would prefer that we request your records from your previous dental practice, please call us. Please let us know when your last dental cleaning was, if you have any specific concerns and the primary need for your appointment so that we can best schedule your appointment to provide you with the care you need.
Take a moment to learn about any symptoms or concerns you may have by reviewing the patient education information on this website. Please make a list of any questions or concerns that you may have. You can also review our staff page and familiarize yourself with the doctors and our dental team.
